Full payment is expected when services are rendered. We accept personal checks, cash, Visa, MasterCard, American Express, Discover, and CareCredit. Returned checks are subject to a service charge.
Helpful Forms to Fill Out in Advance
We recommend that all clients schedule an appointment prior to coming to San Pablo Animal Hospital. Please call to schedule your appointment or complete an Appointment Request Form and we will contact you.
If you are a new client, please complete the New Client Form so that we can establish a new record. You may also complete the form at your first visit, but allow a few extra minutes.
If you need to board your pet, please call and make a reservation first. You may then wish to complete the Boarding Form prior to or when you drop off your pet.
If you would like to drop off your pet for an exam and return later in the day to pick him or her up, please complete the Drop-Off Form and give it to the receptionist when you bring in your pet.
If your pet is scheduled for a surgical procedure or sedation by anesthesia, we will ask you to fill out a Consent Form at the time you drop off your pet. If your pet is over the age of 7, he or she must have a blood profile prior to the surgical procedure. Many people also choose for their pet to have a thorough dental cleaning or be microchipped while under anesthesia. Our receptionists will go over details on these forms at the time of your visit.
